Our goal at Maple City Dentistry is to provide the highest quality dental care possible and to have clear communication in regard to our financial policy.
At the discretion of Maple City Dentistry, ALL ACCOUNTS ARE DUE AND PAYABLE AT TIME OF SERVICE. If a procedure requires multiple appointments, 50% of the fee is required in full at the first appointment and the remaining balance paid in full by the final appointment.

Because instruments, chairs, and personnel are reserved exclusively for your appointment, there is a $35 CHARGE FOR BROKEN APPOINTMENTS LESS THAN 48 HOURS IN ADVANCE.
We will file your insurance claims for you. Patients with insurance are responsible for knowing and understanding their dental insurance coverage/plan. We are not contracted with any insurance so please check with your insurance carrier to see if they cover an “out of network” provider. The patient is responsible for the ESTIMATED non-covered portion, procedures and/or deductibles at the time of the service, OR the patient can sign a credit card authorization to bill their credit card AFTER insurance has paid for the visit. If the insurance company does not pay after 60 days, we will bill you directly for the full balance.
We have partnered with CareCredit to help you finance your dental work. This allows you to complete the dental work without delay and make small, monthly payments to the CareCredit company. CareCredit offers little to no interest payment plans ranging from six months to five years in length.

We also offer Monthly or Bi-weekly payment plans for services over $300 (not to exceed 6 months).
